- Relationship, Full Name, Age, CNIC / B-Form, Work Status, Occupation, Monthly Income (Rs.)
- Add father, mother, siblings, and other family members
Step 2
Education & Finance
C. Educational Information table (add rows)
- Degree / Program, Institution, Board / University, Roll / Reg. No., Status, Year of Study, % / CGPA
- Mark exactly one row as Scholarship For? = Yes for the program you are applying for
D. Financial Information
- Total Family Monthly Income (Rs.)
- Number of Earning Members
- Other scholarship received? (Yes / No, if yes specify)
Step 3
Documents & Declaration
E. Documents to Upload one upload per document
Upload attested photocopies. The form creates an upload field for each item below:
- Passport-size photograph
- Applicant CNIC / B-Form
- Father CNIC / B-Form
- Mother CNIC / B-Form
- Income certificate (salary slip & affidavit)
- Bona fide certificate from current institution
- Degree certificate / transcript for each qualification you add
- Other scholarship proof (if you answered Yes above)
